Corneal Irregularity

Corneal Ectasia

Advanced diagnostics, structural stabilization, and visual restoration for thinning corneas.

Understanding Ectasia

Corneal Ectasia is a group of progressive conditions characterized by the abnormal thinning and outward bulging of the cornea (the clear front window of the eye). Instead of a smooth, round dome, the cornea becomes distorted and steep, causing severe, irregular astigmatism.

Common Causes Include:

  • Keratoconus: The most common form of natural ectasia. It often begins in puberty or the late teens and is linked to genetics, allergies, and chronic eye rubbing.
  • Post-LASIK / PRK Ectasia: A rare but serious complication that can occur months or years after refractive surgery if the remaining corneal bed is too thin or structurally weak.
  • Pellucid Marginal Degeneration (PMD): A condition similar to keratoconus, but the thinning occurs in a very specific crescent shape at the bottom of the cornea.
  • Mechanical Trauma: Chronic, aggressive eye rubbing can physically break down the collagen fibers in the cornea, inducing ectasia.

Corneal Topography
Dr. Fouladian utilizes advanced 3D corneal mapping to detect microscopic thinning and bulging long before it can be seen at a standard slit lamp.

The Daily Struggle of Irregular Corneas

Because the eye is misshapen, glasses and standard soft contacts can no longer focus light properly. Patients with ectasia face unique, exhausting visual challenges.

Daytime Issues

  • Constantly Changing Prescriptions: A feeling that your glasses "never work" or are always outdated.
  • Ghosting & Smearing: Seeing a faint secondary image "smeared" below or to the side of text, making reading exhausting.
  • Light Sensitivity: Increased photophobia and a constant need for sunglasses outdoors.
  • Dryness & Irritation: The bulging cone can cause poor tear distribution, leading to chronic dry eye and a gritty sensation.

Nighttime Issues

  • Severe Starbursts: Every streetlamp and headlight explodes into massive, blinding starbursts.
  • Massive Halos: Large glowing rings around light sources as the pupil dilates and exposes more of the irregular cornea.
  • Driving Anxiety: Extreme difficulty or total inability to drive safely at night due to the overwhelming glare and lack of contrast.
  • Multiple Images: Looking at a digital clock or TV in the dark and seeing 3 or 4 overlapping numbers.

Comprehensive Care

A Two-Pillar Treatment Plan

Dr. Fouladian approaches ectasia by first stopping the disease, and then restoring your vision.

1 Halt Progression (CXL)

If your corneal topography shows that the ectasia is actively worsening, the absolute first priority is to stabilize the tissue.

Corneal Cross-Linking (CXL)

CXL is a minimally invasive procedure that uses Riboflavin (Vitamin B2) drops and targeted UV light to create new chemical bonds across the collagen fibers in your cornea, stiffening it and preventing further bulging.

Specialist Co-Management

Dr. Fouladian does not perform surgery. Instead, he serves as your diagnostic quarterback. He works collaboratively with the most esteemed Corneal Surgeons in Los Angeles. We will expedite your surgical referral for CXL and handle your comprehensive pre- and post-operative care right here in our office.

2 Restore Vision (Scleral Lenses)

Once the cornea is stable, we must fix your vision. Because glasses cannot conform to your irregular eye, Dr. Fouladian personally fits advanced Scleral Contact Lenses directly in our clinic.

The Scleral Miracle

Scleral lenses are large, highly customized rigid lenses. Instead of resting on the delicate, damaged cornea, they vault completely over it and rest gently on the white part of the eye (the sclera).

  • A New Surface: The rigid lens acts as a perfectly smooth, artificial cornea, eliminating ghosting and glare.
  • Liquid Bandage: The space between the lens and your eye is filled with sterile saline, bathing your eye in moisture all day.
  • Extreme Comfort: Because they do not touch the sensitive cornea, they are incredibly comfortable to wear.
