A "Blister" in the Macula

Central Serous Chorioretinopathy (CSCR), often referred to as Serous Maculopathy, is a condition where clear fluid builds up beneath the retina.

Specifically, this fluid accumulates under the macula—the central part of the retina responsible for your sharp, detailed vision. This tiny pocket of fluid lifts the macula away from its nourishing tissue, acting like a blister that distorts everything you look at.

Early Signs & Symptoms

  • Central Blur: A dim, blurred, or darkened blind spot in the direct center of your vision.
  • Micropsia: Objects may suddenly appear smaller or further away in the affected eye.
  • Metamorphopsia: Straight lines (like door frames or text) appear wavy, bent, or distorted.
  • Color Washout: Colors may look dull or desaturated compared to your healthy eye.

One Eye at a Time

Serous Maculopathy typically occurs unilaterally (in one eye at a time). Because the peripheral vision remains intact and the other eye compensates, some patients delay seeking care. However, any sudden distortion in central vision requires an immediate medical evaluation to rule out more severe conditions.

Who is Most Prone to CSCR?

Unlike many retinal diseases that affect the elderly, Serous Maculopathy is uniquely tied to young, active adults and specific lifestyle factors.

Age & Gender

While anyone can develop it, CSCR overwhelmingly affects men between the ages of 30 and 50. It is significantly less common in women and older adults.

The "Type A" Personality

There is a massive, clinically proven link between CSCR and high systemic stress. Patients with "Type A" personalities—those who are highly competitive, deadline-driven, or experiencing intense psychological stress—naturally produce higher levels of cortisol, triggering the fluid leak.

Major Trigger

Corticosteroid Use

Any form of steroid use is the most common chemical trigger. This includes oral prednisone, joint injections, topical hydrocortisone creams, and even steroid nasal sprays for allergies. Steroids alter the permeability of the blood vessels beneath the retina.

Advanced Retinal Imaging

A thorough evaluation of the macula is essential for diagnosing CSCR. Dr. Fouladian utilizes Optomap® ultra-widefield imaging alongside a meticulous dilated fundus exam to physically visualize the fluid blister sitting beneath your retina and rule out other conditions.

Ruling Out Wet AMD

Symptoms of CSCR are nearly identical to "Wet" Macular Degeneration. Dr. Fouladian's urgent evaluation is required to ensure the fluid is from a stress-induced leak, and not from actively bleeding, destructive blood vessels.

Treatment Options

  • 1. Observation & Lifestyle Modification In many acute cases, the body will reabsorb the fluid on its own within 1 to 3 months. Dr. Fouladian will monitor your macula closely while advising you to reduce caffeine, manage stress, and (with your PCP's approval) discontinue any corticosteroid medications.
  • 2. Medical Therapy We may prescribe certain oral medications (like Eplerenone or Spironolactone), which are known to help dry up the subretinal fluid in chronic cases.
  • 3. Retina Specialist Co-Management If the fluid does not resolve after several months, or if it is severely threatening your vision, Dr. Fouladian will refer you to an esteemed Retina Specialist. They can perform advanced treatments such as:
    • Photodynamic Therapy (PDT): A "cold" laser treatment combined with an IV medication to safely seal the leak without scarring the macula.
    • Micropulse Laser: Gentle laser therapy to stimulate the pump cells under the retina to clear the fluid.
